Housing Placement Coordination

We coordinate placements for:

Complex Placement (special restrictions, urgent needs, or high coordination)

Transportation Coordination

Transportation is often one of the biggest barriers to successful placements.

Rise coordinates reliable transportation through our network of trusted transportation partners.

We help ensure individuals get safely from hospitals, detox centers, shelters, or community locations to their housing or treatment placement.

Transportation Scheduling Fee

$50 per ride

This includes:

• Coordinating with transportation providers

• Scheduling pickup and destination

• Communication between agencies and drivers

• Confirming arrival and completion

Transportation companies work with Rise to provide consistent rides across Arizona.

Intake Paperwork & Documentation Support

Many placements are delayed because of incomplete intake paperwork.

Rise helps agencies and clients complete required documentation quickly so placements can move forward without unnecessary delays.

Services may include:

• Intake paperwork assistance

• AHCCCS enrollment guidance

Rise Homes ONLY:

• ✔ Matches clients to available housing vacancies.
• ✔ Coordinates with third-party transportation providers.
• ✔ Manages ride scheduling and logistics.
• ✔ Facilitates communication between providers and families.
• ✔ Ensures accurate coordination and placement documentation.

Rise Homes does NOT:

• ❌ Provide clinical case management or treatment.
• ❌ Provide direct transportation (we are a dispatch/coordination hub).
• ❌ Provide on-site monitoring or wellness checks.
• ❌ Provide long-term clinical support or medical services.

We offer tailored contracts for agencies specializing in:

• Behavioral Health & MAT Clinics
• Re-entry & Youth Transition Programs
• IOP / PHP / IOCC Programs
• The Motherhood Network (Adoption & Prenatal Specialty)

We offer flexible payment structures for providers, families, and case managers:

• Digital: Zelle, PayPal, and Private Pay.
• Institutional: Monthly Invoicing (Available for contracted providers only).
• Agency-Led: Coordination with AHCCCS Case Managers.

*Basic Behavioral Health referrals are covered by our provider network; Managed Logistics (The Motherhood Network) is a premium contracted service.
