We are not a treatment center, group home, or transportation company.

We are a coordination network.

Housing Match & Placement – $250 One-Time Fee

A one-time service connecting clients to safe, available, behavioral-health-friendly housing options.

Includes:

Intake & needs assessment (5–10 minutes)

Match with vetted housing partners

Availability confirmation

Move-in coordination

Communication with provider/family

Documentation delivery (placement confirmation)


No ongoing support included.
Placement only.

For urgent placements or same-day/next-day requests.

Includes everything in standard placement PLUS:

Same-day outreach to housing partners

Faster match confirmation

Direct communication channel for urgent updates

Rise coordinates rides through trusted third-party transportation providers, ensuring clients get to appointments reliably.

Transportation Match (One-Way) – $25 Coordination Fee

Client or provider pays the driver separately.
Rise only handles the match + scheduling.

Includes:

Scheduling

Pickup coordination

Communication with driver & provider

Confirmation message sent to provider/client

Round Trip Coordination – $45 Coordination Fee

For appointments needing both drop-off and return pickup.


Weekly Transport Coordination Pass – $40/week

Covers Rise coordination ONLY for up to:

5 one-way rides OR

3 round trips


Driver fees still paid separately to the transportation provider.


Monthly Transport Coordination Pass – $150/month

For agencies that schedule frequent rides.
Includes coordination for up to 20 rides/month.

Housing Match + 4 Ride Coordinations – $300

Includes:

One housing placement

Coordination of 4 rides (to intake, IOP, probation, etc.)


Rides are paid to the driver separately.


Housing Match + 10 Ride Coordinations – $350

Best for IOP, IOCC, and early recovery schedules.

For agencies placing 10+ clients per month.

Housing Match: $200/client

Transportation Coordination: $20 per ride

Monthly Coordination (High-Volume): $100 per client (up to 20 rides scheduled)

IMPORTANT CLARIFICATION

Rise does NOT:

❌ Provide case management

❌ Provide transportation directly

❌ Provide monitoring or wellness checks

❌ Provide treatment or clinical services

❌ Provide long-term client support
