RISE Homes AZ and The Motherhood Network (collectively "RISE") operate strictly as a centralized placement and logistics coordination hub.

• Non-Provider Status: RISE does not own, manage, or operate residential housing facilities, sober living homes, or transitional housing.
• Transportation: RISE does not own a fleet of vehicles and is not a direct transportation provider. All transport is executed by independent, third-party vendors.
• Clinical Disclaimer: RISE does not provide medical, clinical, or behavioral health treatment. We do not provide case management, wellness checks, or crisis intervention.

Rise (“Rise Homes,” “Rise Network,” “we,” or “our”) operates as a matching and coordination service that connects clients, families, case managers, and providers to housing partners and third-party transportation 

We are NOT a housing operator, behavioral-health facility, sober living home, treatment provider, or transportation 

By utilizing our coordination services, agencies and partners acknowledge the following:

• Third-Party Actions: RISE is not liable for the actions, omissions, or service quality of third-party housing providers or transportation vendors. Any disputes regarding residency or transport execution must be handled directly with the respective service provider.
• Safety & Outcomes: While RISE maintains a rigorous vetting process for our network, we do not guarantee specific client outcomes or the absolute safety of third-party environments.
• Information Accuracy: RISE relies on the data provided by referring agencies and housing partners. We are not responsible for placement delays or errors caused by inaccurate or incomplete information submitted to us.

We collect only the information necessary to execute the 2-Hour Standard for placement and transport.
​Our Data Promise: We do not sell, rent, or trade personal or professional information.
​Information Collected: We collect professional contact details, client demographics, non-clinical behavioral-health needs, and logistics schedules.
​Authorized Sharing: Data is strictly shared only with the specific housing partner, transport vendor, and referring agent involved in a specific placement loop.

​Network Integrity: RISE reserves the right to remove any partner from our network for violations of our Integrity Policy, including unsafe housing conditions, unprofessional conduct, or discriminatory practices.
​Non-Discrimination: RISE facilitates placements in compliance with Fair Housing standards and does not discriminate based on race, color, religion, sex, disability, familial status, or national origin.

Coordination Fees: RISE charges a professional fee for management and logistics.
​Pass-Through Costs: For specialized programs (such as The Motherhood Network), transportation costs may be billed as pass-through expenses to our third-party vendors.
​No Interference: RISE does not take a percentage of the housing provider's base rent or revenue. Our fees are strictly for the coordination of services.

Rise does NOT operate or manage housing.

All housing sites in our network are run by independent homeowners, organizations, or providers.

By submitting a housing request, you acknowledge that:

Rise does not inspect homes daily or guarantee living conditions.

Rise is not responsible for rules, policies, fees, or operations of any housing provider.

Rise does not determine who is accepted by a housing provider—final decisions are made by the provider.

Client behavior, compliance, and safety inside the home are the responsibility of the client and the housing provider.

Any housing disputes must be addressed directly with the housing provider—not Rise.

Communicate with approved providers

Share necessary non-clinical information with housing & transport partners

Coordinate services on your behalf

Our facilities are designed to provide a safe and comfortable environment for our residents. We offer a range of amenities, including private and semi-private rooms, a fully-equipped kitchen, a communal living area, and outdoor space for relaxation and recreation.

Rise provides coordination services only, which begin as soon as a request is submitted.

Because coordination work is completed immediately, all coordination fees are non-refundable, including:

Housing match fees

Transportation scheduling fees

Bundle fees

Provider invoicing fees


If Rise is unable to locate a match, we will offer credit toward a future placement, not a refund.
