Rise was created to simplify one of the most difficult challenges professionals face when supporting individuals during times of transition: coordinating safe housing placements and reliable transportation quickly and efficiently.
Hospitals, case managers, behavioral health providers, adoption professionals, and community organizations often spend significant time contacting multiple providers, confirming availability, coordinating transportation, and managing intake requirements. These situations frequently require urgent coordination, yet the process can be fragmented and time-consuming.
Rise was established to streamline this process.
As a centralized coordination network, Rise helps agencies connect with housing providers and transportation partners through one organized system designed to manage the logistical side of client transitions.
Rather than navigating these complexities independently, professionals can rely on Rise to coordinate placement outreach, confirm availability, and assist with transportation logistics through our growing provider network across Arizona.
Our Operational Model
Rise functions as a placement and logistics coordination hub.
We do not own or operate housing programs, transportation companies, or treatment facilities. Instead, Rise works alongside independent providers and agencies to coordinate the final steps of a client’s transition into appropriate housing and supportive environments.
Our role is to organize the process, manage communication between stakeholders, and reduce the time professionals spend navigating multiple providers.
This structure allows agencies to remain focused on clinical care and case management while Rise manages the coordination required to secure safe placements.

Transportation Coordination

Rise coordinates transportation through vetted third-party vendors, helping ensure individuals are transported safely and efficiently to their destination.
Transition Support Coordination
Rise also assists professionals by managing the logistical details that can delay placements, including communication between agencies, providers, and transportation partners.
By organizing these elements within one coordinated system, Rise helps reduce discharge delays, administrative burden, and placement uncertainty.
The Motherhood Network
Rise also operates The Motherhood Network, a specialized coordination initiative designed to support adoption professionals and organizations working with birth mothers.
Stabilizing housing and transportation during pregnancy is one of the most common logistical challenges adoption professionals encounter. The Motherhood Network helps address this need by coordinating housing placements, transportation scheduling, and documentation support through Rise’s provider network.
This structured approach helps agencies provide stability and support for birth mothers while reducing the coordination burden on agency staff.
Tyler’s Path
Rise also supports Tyler’s Path, a nonprofit initiative created in memory of Tyler.
Tyler’s Path was established to honor Tyler’s life by helping expand access to housing opportunities, coordinated support, and resources for individuals navigating difficult life transitions.
Through this initiative, Rise seeks to strengthen community partnerships and support programs that help individuals find stability, connection to resources, and pathways toward a more secure future.
