We do not own or operate housing programs, transportation companies, or treatment facilities. Instead, Rise works alongside independent providers and agencies to coordinate the final steps of a client’s transition into appropriate housing and supportive environments.

The Motherhood Network helps address this need by coordinating housing placements, transportation scheduling, and documentation support through Rise’s provider network.

Tyler’s Path
Rise also supports Tyler’s Path, a nonprofit initiative created in memory of Tyler.
Tyler’s Path was established to honor Tyler’s life by helping expand access to housing opportunities, coordinated support, and resources for individuals navigating difficult life transitions.
Through this initiative, Rise seeks to strengthen community partnerships and support programs that help individuals find stability, connection to resources, and pathways toward a more secure future.
